Please note: this form does not require any information about your team members, your team name, or when your team will take the test.

Point Person: The person filling out the form should be an adult, and provide their email address in the first field. This person is the “point person” for the team. They will receive an Onboarding Email that contains instructions before the test. This information will be sent to the Point Person’s email, and they can forward this email to the parents of the other team members.
